The MHRA currently runs five reporting systems to collect reports of different types of incidents involving medicines, medical devices and blood. These reports are vital to help the MHRA identify safety problems, however we know that not all incidents are reported.

 

Following initial discussions with a small number of stakeholders we are considering whether bringing all five systems together under the Yellow Card Scheme brand (currently used for reporting side effects to medicines) could help increase incident reporting and reduce confusion of reporters.

 

The Yellow Card Scheme is a well-established reporting system having been in place for nearly 50 years. There is also understood to be a high level of awareness of the scheme among a significant proportion of healthcare professionals.
